For loans today, most underwriting models use the "File" score. This is the lowest middle score of the borrowers. If her score is 620, then that COULD be the reason. HARPS can be tricky. If may not be the score alone. I've had Fannie harps with scores below 620, but the borrower needed to show reserves over $100,000 to get the approval. They had it so we got the approval. Many times lenders minimize the amount of data they input because sometimes it is overkill. If your lender didn't use all of your assets, that could impact your approval. The other thing is what are the issues in your wife's credit that caused the 620 score.
