There is a specific form required - "Request for a Certificate of Eligibility." It is also known as VA Form 26-1880. You'll need to complete this form and send it to the VA Eligibility Center in North Carolina along with a copy of your DD-214 Member Copy 4. You'll find the instructions attached for your assistance. Plan on receiving it 45-60 days after application submission.
