does the PUD rider require witnesses

A PUD ride is usually signed and witnessed by the person performing the loan document signing. If for some reason the rider was not included in the original loan package and the lender has sent you one for signatures, I would say that the answer would be no, it does not need to be witnessed nor notarized, UNLESS the lender is requiring you to get it witnessed or notarized.
