Its always a good idea to have one done, they can uncover things that the appraiser is not required to inspect or comment on. A home is the largest investment most people make in their lives, it only makes sense to protect it. If you choose to get one look for a inspector that is also a certified engineer they will better be able to asses the structure and typically are about the same price

When you get an FHA loan you will get an appraisal that meets FHA standards. There is not a separate FHA inspection that is done beyond the appraisal. A home inspection is not required and you don't have to get an inspection if you don't want to. However, I would highly suggest getting one as an inspector looks at things much closer than an appraiser does and may find things that are needed or damaged. I would talk with your real estate agent and go over your concerns if you have any, but it is not required for the home loan. Unless you have to get a termite inspection and that is a different story.
