Can one do a VA cashout with no appraisal? I see it only offered on the streamline

You can't receive cash if you proceed with a streamline. A VA loan allows to 100% loan to value.

No. A Streamline refinance is allowed with no appraisal because VA is already on the hook for the loan amount outstanding. If you want to pull cash out you have to prove your home has the value to warrant it.
